Residence Cards or Proof of Registration for EU and EEA Nationals

The application form for an EU/EEA residence card or proof of registration is OD1 Application for EU-residence certificate.

Your application should be submitted (after arrival in Denmark) to the local Statsforvaltning in the area where you will be living.

If you know the exact address where you will be living you may also apply by post through the Embassy. Please note that if you apply through the Embassy you must take up residence in Denmark before the Certificate can be issued.

The following is required for an application

1. 2 completed application forms OD1. Both forms must be identical. Please provide your UK contact details: address, mobile telephone number and e-mail address.
2. Passport (copies of passport pages with your personal information accepted)
3. 2 colour photographs (lifelike, facing the camera measuring 35 x 45 mm).
4. The relevant documents as listed under Section 2.
